Employers that sponsor health plans for employees are dealing with increasing costs and the challenges of providing quality care at affordable prices. One of the approaches that employers are considering is moving primary health care to the workplace by means of onsite (or near-site) clinics for employees and their families. Similarly, the addition of electronic means of communication between patients and medical professionals (telemedicine) is becoming more common. Both can help health plans save on costs and, if properly structured and administered, can provide quicker and more economical care for employees. But implementing either approach requires that the onsite or telemedicine coverage be coordinated with other coverage offered by the employer. For example, onsite clinics and telemedicine can render employees ineligible for High-Deductible Health Plans (HDHPs) and Health Savings Accounts (HSAs), or might inadvertently create a health plan that must comply with the ACA (including potential excise taxes and penalties), or might constitute an Employee Assistance Plan (EAP). The use of telemedicine for mental health care may raise problems under the Mental Health Parity and Addiction Equity Act (MHPAEA).
